I think that would all come down to the design, safety features employed, and self-preservation of the operator? As long as it's all within the weight capacity of the trailer/lift, the trailer is leveled/stabilized with jacks, the car is placed on the lift so it wont fall, and the lift has safety catches in case it fails, it seems about as safe as any other lift with a ~3000lb car on top.
I'm not sure I'd want to be doing a transmission swap on top of a trailer anyway, but being able to take a lift anywhere you take your car could have its advantages.
I've seen similar trailer-mounted lifts on UK TV programs. IIRC, they are often used by mobile MOT inspectors. I've seen some really elaborate ones in custom enclosed trailers.
Given the car on the lift has a Euro front plate, that whole ad looks a bit fishy. Unless maybe the seller has access to used ones in the UK and is importing them into New Orleans.

The tow vehicle also looks like a Land Rover Discovery, which is not exactly a common choice over here (this crowd nonewithstanding), but is a very common choice in the UK. Not to mention that it's parked on the correct side of the road if you're in the UK.

I used to work for a mobile communications/specialty vehicle shop and we designed & built many of these things for US customs for scanning purposes.
