I'm guessing it has to do with the previous engine grenading and shearing the crank at the 4/5 journal causing those pistons and rods to beat themselves and the block to death. Shadetree "mechanic" who put the heads and cam into the junkyard explorer block apparently didn't take the heads to a machine shop and have them checked and just assumed that things were good.
