My RX-7 rolled out of the driveway in gear with the parking brake deployed... kind of. I'd cranked the parking brake on, but when the brakes cooled down a bit, it became clear I'd not engaged it all of the way and it rolled down my steep driveway and took out my mailbox.
having had saabs for a few years now.. my BMW also gets left in reverse now along with the handbrake.
Both, but due to the reverse lock out spring in the MS3 i just leave it in 1st or 2nd
Both the gear and the parking brick.

In gear + hand/parking/emergency brake.
Automatics in P with the hand/parking/emergency brake actually holding the car. I hate it when someone parks an auto w/out the brake and it rolls a bit, causing one to have to hank on the gear selector.
Both.
And I've still had a car roll into a snowbank...

That was when I found out that the wankel doesn't hold compression like a piston engine. I didn't have the parking brake on.
My bad.
1st gear, parking brake on.
Crap. I need to take a pic of my wood block I use for my cars that dont have a working parking/e brake. I call him Ken Block. I park on ken block all the time.
well my latest 240sx has a working e brake and so does my Z.
I usually use both, but on a level surface I may not use the e brake. But the majority of the time I do.

Back when I was in high school, I was in the car about to head to work, but I realized that I forgot something in the house. I ran in to grab whatever it was, lunch or a CD. Thirty seconds later, when I opened the door to the garage, the car was not where I left it. No, it was thirty feet away from where I parked it, a few feet downhill, parked on top of a neighbor's small tree. That's the one time I forgot to put the car back in gear and re-engage the parking brake. I now use both in case I forget either.
We don't have much in the way of hills around here, so I just put them in gear. Either first or reverse or the automatics in park. We call the "parking brake" the emergency brake around here. I've had the cables on more than one of them freeze up from lack of use. The Abomination doesn't even have one.

It's quite a rude awakening to get up one morning, go out to the driveway and find your car at the bottom of the hill... Fortunately, no damage was done on it's way. Ever since then I use the park brake and put the car in gear if it's a manual and 'P' if it's an automatic, which I think goes without saying.
Unfortunately, my low compression turbo motors don't generate enough compression to hold the cars even on shallow grades.

Both. I've had so little faith in the "parking brake" on most of the cars that I've owned that I call it the "handbrake" instead. 1st Gear engaged when parking uphill, reverse engaged when parking downhill.

First gen VW Rabbits had a recall that warned owners to NEVER leave it in gear.
But that was because the radio antenna gasket would fail and drip water on the fuse block, starting the car.
Of course, you were supposed to get it fixed ASAP.
I leave my A/T truck in Park and rarely use the parking brake. I know it's wrong. I feel so ashamed.

I tend to only use the parking brake... unless the parking brake has proven to be unreliable or prone to sticking, then I'll park in gear. Of course, only using the brake has bitten me on occasion... a few weeks ago with very expensive reprocussions...

Depends on the car. if it's one I want to keep, in gear brake on. In an auto, in park, if it's a console e-brake, brake on. if it's a car I want to get rid of, in neutral, on a steep hill, no brake, or, running with the keys in it outside a liquor store in the hood.
Parking brake only on flat ground. Both on a hill.
Same for me. It's mostly flat here in Melbourne, so I only leave it in gear occasionally.
Even though its incorrect I generally leave the car in neutral and use the parking brake. Neither of my cars have a clutch safety switch and I sometimes forget that I happened to leave it in gear. Also my Supra has a heavier than stock pressure plate so I would have to take it out of gear before starting it anyway because it can cause excessive thrust bearing wear.

Your post made me wonder..has anyone else ever noticed that when we modify our cars, the procedures required to do simple stuff like start them & park them get more complicated? It sounds like the stuff that my aviation friends talk about when a pilot has to be "type rated" for a bigger aircraft (not dissing you Wes, I've had cars like that too!).

Facing downhill; reverse w/ parking brake.
Facing uphill; 1st w/ parking brake.
Auto...I rarely use the parking brake, usually only when it is up on ramps. I always use a chock on ramps too.
