Two aging grandparents. They need each other for everything, and can't easily get attention when they are across the house. I need a "help me I've fallen and I can't get up" thing that they can use just between each other, perhaps something which is connected to a bluetooth earpiece.
Someone has solved this problem before me. How did they solve it?

Baby monitor with a talk back function. It's not perfect but readily available and relatively cheap. Very easy to use.

They need to each wear the thing, and it can't be a cell phone. Ideally, they each have a thing in their ear, they press a button and talk and the other person hears it. That's really all I want.

A PTT P2P VoIP app for their phones could work, if they normally carry smartphones around. They're often advertised as "wifi walkie talkie" apps. There are free ones for Android.

Yeah no. Nothing that requires a smartphone. The one-button nature of the other product is a huge attraction. My Grandma still doesn't understand how to use a thermostat.
You could just buy them...walkie talkies. Seems to meet all of your requirements. Plugin earbuds would probably even be easier for them to figure out how to reconnect than bluetooth.
What about a couple of Amazon Echo dots? Alexa, wake up grandpa.

Nothing connected to a phone or walkie talkie. It will inevitably get lost or they won't know how to use it.

Well now you're changing the requirements on us. You were the one who brought up connecting it to a bluetooth earpiece.

The issue is, you'll not get an ear piece with enough battery life or power/range to reach more than a few feet to another device like a phone or a base station. That's just the way they are designed and limited due to FCC rules.
So a hand-held walkie talkie with an ear piece, a wrist based walkie talkie, a cellphone with a headset, a VOIP phone solution with a wireless headset and base station.
The VOIP phone could work well, but it won't be cheap or easy to configure, but I suspect it could be setup to simply open a line between the two "phones"

Seriously, though, does it need to be a voice communication device, or do they just need to have some way to alert the other person that there is an emergency? What about a wireless doorbell mounted centrally and give each of them a button on a lanyard?
The Echo Dot is pretty cheap, and it can hear well from pretty far away, and you can talk to other Dots in other rooms. It plugs into a wall, nothing to charge or carry, and you can talk to any other Dot in any other room.
