Every Centurion I've ever seen is a dually.

Its too new to be a Centurion, and the sliding door makes it look totally home made.

Yup. Van conversion places used to make with dually axles for 5th wheel trailers. This one looks like a homemade job gone wrong.
Company called Centurion made them in the 70s & 80s. There may have been others. Popular with grandparents with 5th wheels and ranchers with horse trailers.
Had a neighbor that hauled his 5th wheel and 4 kids with an older Centurian for years. Must of been a tough workout for a wheezing 302 smog motor.
What does that side door do? I did not call it a sliding door as I don't believe it has that capability anymore!
