Does this sound like it just needs a tensioner? Or are there bigger issues?

Tensioner and guides, yes. If you are an optimist you could just slap a new tensioner on there in a few minutes.

Cam too advanced could be simply the camshaft position sensor. I'm still betting that it is the VANOS though.

The BMW V8s are known for being hard on timing chains if the oil isn't changed religiously. I'll take "timing chain, tensioner and guides" for $500, Alex.
