It can vary a lot from state to state. I have bought plenty of cars with salvage titles and registered them here in Oklahoma. If it is at least 10 years old, practically ANYTHING can get a rebuilt title. Newer than 10 years needs to have repairs verified by an inspection.
When I got tags and rebuilt title for this Camry they just looked at the VIN and recorded the mileage.
I've insured with USAA and another regional company for a couple decades and never had any questions about covering a 'rebuilt' vehicle with both liability only and full coverage. As a seller I generally expect 30% less value and a much smaller pool of buyers on a car that's been repaired. At the 'fully depreciated' end of the market, I don't see the title making a difference - if it runs and drives and has a streetable title, it sells for the same as Bubba's clean title hooptie.
