Let's assume I had a Fiat 124 minus an engine/transmission. Let's further assume I wanted to put a GRM Italian V6 into it with a manual transmission for Challenge-like money. What's the suggestion?

I can't think of any Italian V6 that will be plentiful enough to be cheap and doesn't have a funky transaxle scenario like a Milano or GTV-6.
So I vote for VR6. Call this guy and find out what he did for a transmission. The noise coming from a VR6 would be absolutely biblical once placed in an old Italian roadster.
Find a crashed Ducatti, and make it bike-powered...
As mentioned earlier, Italian sixes may be hard to find for Challenge money. That's why I'm still deciding between the twin-cam Lancia 4-banger I have for my Fiat, or just go with a sport-bike engine... Or two.
Doesn't HAVE to be Italian, I guess, but I was just trying to be consistent. I considered a Biturbo as they pop up cheap now and then and nobody seems to want the engines.

The answer is often, bike power. I concur. Easier than you'd believe too.
There are some alfa spiders with an alfa 12v v6 in them. I dont think a v6 would fit in a 124 very well. A VR6 might actually work though, since its nowhere near as wide. There is no way a biturbo engine will fit a 124 without a lot of cutting, they are really wide.

the "problem" with those is that you have to make a nice adaptor to fit the trans OR you get an Alfa 6 from South Africa and use it's trans.
Sadly, while it's easy to get an Alfa V6 for Challenge money, it's far from it to get the V6 + a RWD trans attached to the back of it.

I don't know what they cost but the sound is fantastic and they appear to be compact.
